Without expectation of compensation or other remuneration, now or in the future, I hereby give my consent to the Ridhwan Foundation and the Diamond Approach Online to use my image and likeness and recorded statements from me in its publications, course materials, advertising or other media activities (including the website and the Internet).
This consent includes:
(a) Permission to film, photograph, tape, or otherwise make a video reproduction of me and/or record my voice during the “Meditation and the Diamond Approach” seminar, which was held on Zoom videoconference on Sunday, March 10th, 2019 via video/tele-conference.
(b) Permission to use my name as it appears in the video (or audio) either on-screen (as my Zoom username) or as spoken by me, the teacher, the facilitator or other participants.
(c) Permission to use the discussion in entirety or excerpts or quotes from the discussion, the video or audio recordings, transcripts, photograph(s) or stills from the recording, and/or recording of my voice, in part or in whole, in its publications, including in email, electronic media (including the Internet), in course materials and/or in mailings for education and/or awareness.
I understand that the Program may be broadcast and distributed without limitation through any means and in perpetuity without compensation to me of any form or type. However, I acknowledge and agree that the Foundation is under no obligation to broadcast or distribute the Program.
Unless formally revoked by me at some point in the future, this consent is given in perpetuity.
To revoke this consent, I must give written notice via email to daosupport@ridhwan.org, and I understand that it may take up to 60 days for my name or likeness to be removed from the video and audio recording, and the recordings will remain available in their original state in the interim, and that the Ridhwan Foundation will not have the ability to control or edit previously downloaded copies of the video or audio by other course participants.
